For eligible Medicaid clients with a history of significant mental health challenges (e.g. Bipolar Disorder, Schizophrenia, OCD, Depression) and one or more hospitalizations, Pesach Tikvah's CORE program (Community Oriented Recovery and Empowerment) and HCBS program (Home & Community Based Services) offer a customized array of services based on each client's needs. Providers can meet with clients in their homes or community settings. Services provided include job counseling and ongoing workplace support, educational support, daily living skills support, peer support, in-home counseling, and social skills counseling.

CORE providers work individually with members at their homes or in the community, in flexible locations, meeting them where they are. CORE supports individuals in moving past their challenges and toward their goals.
  • Psychosocial Rehabilitation Service (PSR): Building skills such as time management, household management, daily living skills, communication/socialization/relationship skills
  • PSR with Employment Focus: Providing support and skills-building through the employment seeking process and on the job support
  • PSR with Education Focus: Providing support and skills-building for members with education goals, such as obtaining a GED, getting peer certification, completing college, taking computer courses, etc.
  • Peer Support: Role modeling provided by staff who have experienced mental health challenges and can use their life experience to provide practical support and guidance
  • Community Psychiatric Support and Training (CPST): In-home or telehealth therapy provided by licensed mental health providersfor people who are generally unable to attend in-person sessions at a clinic

CORE's Service Providers offer hope, support, recovery, and skills. There are Service Providers who are fluent in English, Yiddish, Spanish, Hebrew, and Russian.

Eligibility

CORE support services are open to adults aged 21 and older, who have a HARP (Health and Recovery Plan) plan under Medicaid. The services are geared to those with serious mental health challenges, who may have experienced one or more hospitalizations.
